📋 Tanggung Jawab Pekerjaan

Develop and implement comprehensive advance care plans for patients in collaboration with healthcare teams.,Educate patients and their families about their healthcare options and preferences.,Coordinate with healthcare providers to ensure the implementation of advance care plans.,Monitor and evaluate the effectiveness of advance care plans.,Provide guidance and support to healthcare teams in delivering patient-centered care.,Stay updated with the latest advancements in healthcare and advance care planning.,Participate in team meetings and contribute to the development of hospital policies and procedures related to advance care planning.

🎓 Kualifikasi & Syarat

Bachelor's degree in Nursing, Medicine, or a related healthcare field.,Minimum of 5 years of experience in healthcare, preferably in a leadership role.,Certification in advance care planning or a related field is a plus.,Strong communication and interpersonal skills.,Ability to work effectively in a team environment.,Excellent organizational and time management skills.,Compassionate and patient-centered approach to care.,Familiarity with healthcare regulations and policies.
